If you’re adopted, were born at home with a midwife or can’t remember every single job you have ever had and can’t write down the addresses of every single place you’ve ever lived, you may have serious trouble getting a new USA passport if you are asked to complete this new “proposed” form…

According to the proposed form: “Failure to provide the information requested may result in the denial of a United States passport … ”

It’s probably safe to assume that if you submit the same kind of poorly Photoshopped birth certificate that President Obama (see video below) surprised us with earlier this year with your new passport application, you will never be issued a new passport.

In March 2011, the State Department published a notice in the Federal Register that they were proposing a new “Biographical Questionnaire” for some passport applicants.

This proposed new Form DS-5513 asks for all addresses since birth; lifetime employment history including employers’ and supervisors names, addresses, and telephone numbers; personal details of all siblings; mother’s address one year prior to your birth; any “religious ceremony” around the time of birth and they want you to confirm if your were circumcised or not???

The laughable Privacy Statement reveals that your “private” information will not be kept private at all and will in fact be given to every Tom, Dick and Harry at home and abroad if they feel like it:

“The information solicited on this form may be made available as a routine use to other government agencies and private contractors to assist the U.S. Department of State in adjudicating passport applications and request for related services, and for law enforcement, fraud prevention, border security, counterterrorism, litigation activities, and administrative purposes. The information may be made available to foreign government agencies to fulfill passport control and immigration duties. The information may also be provided to foreign government agencies, international organizations and, in limited cases, private persons and organizations to investigate, prosecute, or otherwise address potential violations of law or to further the Secretary’s responsibility for the protection of U.S. citizens and non-citizen nationals abroad…”

Privacy-Act-Statement

According to the Federal Register website, in their 60-Day Notice of Proposed Information Collection: DS-5513, Biographical Questionnaire for U.S. Passport, 1405-XXXX:

“We are soliciting public comments to permit the Department to:

  • Evaluate whether the proposed information collection is necessary for the proper performance of our functions.
  • Evaluate the accuracy of our estimate of the burden of the proposed collection, including the validity of the methodology and assumptions used.
  • Enhance the quality, utility, and clarity of the information to be collected.
  • Minimize the reporting burden on those who are to respond, including the use of automated collection techniques or other forms of technology.

Abstract of proposed collection: The primary purpose for soliciting this information is to establish citizenship, identity, and eligibility for a U.S. Passport Book or Passport Card. The information may also be used in connection with issuing other travel documents or evidence of citizenship, and in furtherance of the Secretary’s responsibility for the protection of U.S. nationals abroad.”

Does anyone with more than one brain cell actually believe this?

The State Department has still not made it clear who exactly must complete this new, proposed form but it’s safe to say that based on the government’s track record of lying to it’s tax paying citizens – all of the time – that a lot more people will be asked to complete the new Form DS-5513 form than we are being led to believe.

Most mature adults who’ve lived on planet Earth for more than 40 years would find it nearly impossible to provide such detailed information, yet the form requires that your answers “are true and correct” under penalty of imprisonment.

No matter how hard it may be for you to believe right now, in the not so distant future – probably based on some upcoming fabricated “terrorist” event like 9/11 – it wouldn’t surprise me in the slightest if US citizens will have to apply to retire outside the USA “for your protection.”

They’re obviously working on it so if you haven’t ever applied for a U.S. passport, it might be wise to do that now.
